order form for credit card. So, you print out the form, and fill out the card

You fill out the information to the order sheet and submit, then please send
the check in Japanese Yen to our company.
By telegram transmission
you fill out the information to the order sheet and submit. After we check your
order sheet, we will inform you(to your E-mail) the bank account where you should pay.
Important:Payment is acceptable only in Japanese Yen.
